Eligibility Criteria:
Each recipient is expected to attend the awards ceremony to accept his or her award, except in extraordinary circumstances.
Please submit a separate form for each person you are nominating. Submit one form for couples or a family nomination.
While letters of recommendation and extra materials aren't required, they help the committee see the full scope of the nominee impact. The quality of the information provided is more important than the quantity or relative prominence of the writer(s).
